Health visits / advocacy services / interpreting
A health advocacy service is provided to support people with understanding and attending outpatient appointments. This may include advocating on a patient’s behalf to schedule an appointment at a convenient time, support with transport to and from an appointment, helping to understand where the patient needs to go, interpreting what their appointment is about, and providing reminders for attending appointments.
The team works individually with patients to address these issues and ensure that they attend their appointment. Between each appointment, the team can arrange for patients to receive regular health checks from our nursing service and this can be a home visit or when attending one of our health promotion sessions.
Health visit / advocacy / interpretation services focus on:
- Transport support
- Scheduling appointments at convenient times for the patient
- Providing a support person at an appointment
- Helping the patient to find the location of an appointment
- Interpreting / explaining what the appointment is about
- Appointment reminders
