Home based support service
Home based support services aim to provide positive outcomes for ‘at risk’ individuals and families who require assistance to a range of health and disability support services, and who for various reasons are unable to access those required services on their own. Services advocate and encourage independence for individuals and families.
Home-based support workers conduct home visits and assess individuals and families for a range of issues such as health problems, housing problems, income issues, unemployment, truancy, sole parenthood, high transience and offending.
Home based support services focus on:
- Needs assessments
- Monitoring and evaluating progress
- Information, advice and education
- Support and advocacy
